Do You Need a Referral for an X-Ray, Ultrasound, or Mammogram in Toronto?

Most diagnostic imaging exams in Ontario require a requisition, also called a referral, from a healthcare provider before you can be scanned. Here is when you need one, who can write it, and what to do if you do not have it yet.

Who Can Write a Requisition Form

At Annex Medical Imaging, we accept requisition forms from a physician, chiropractor, midwife, or nurse practitioner, for the exams each of these providers is authorized to request. Bring your requisition and a valid OHIP health card to your appointment so your exam can be billed correctly.

Exams You Can Book Without a Referral

Not every exam requires a doctor’s requisition. If you are between the ages of 40 and 74, you can self-refer for a routine screening mammogram through the Ontario Breast Screening Program (OBSP), as long as you have no acute breast symptoms, no personal history of breast cancer, no mastectomy, and have not had a mammogram in the past 11 months. How to Send Us Your Requisition

You can bring a paper copy of your requisition with you, or send it ahead of time. We accept requisition forms by email at reception@annexmedicalimaging.com (a PDF copy is fine) or by fax at 416-531-1127.

What Happens If You Do Not Have a Requisition

Without a valid requisition, most exams cannot be billed to OHIP and cannot proceed. If you are not sure whether your paperwork qualifies, call us at (416) 531-1128 before your visit and we can confirm what we need.
