Sinus X-ray’s – X008 – $60.00
Chest x-rays for Immigration purposes – X090 – $60.00
Consultation reports for Chiropractic exams – A335 – $15.00
High Risk Bone Densitometry (within 1 year of last exam) – X155 – $200.00
If you have any of the above uninsured examinations, you will be required to sign a consent form giving us consent to go ahead with the examination and that you also agree to pay the associated fee.
